Chet Whitaker Memorial Scholarship Foundation

Submit Your Application

Applications can be submitted online or printed out and scanned in. ALL required attachments, except a letter of recommendation, must be submitted at the time your application is submitted. DO NOT SEND materials separately. Only those applications filed on time will be given consideration.

Your complete application, including all attachments, can be submitted electronically as a PDF or can be emailed to brian@chetslegacy.org with the name of the scholarship in the subject line of the email. Deadline for filing is January 31, 2025. Letters of recommendation must be sent by mail and postmarked no later than January 15, 2025. Incomplete or late applications will not be considered.

Your application will be reviewed by members of the Scholarship Committee and the fund holder. Selection will be based on a student’s overall leadership skills, sportsmanship, and dedication (as judged by the coaching staff), overall academic progress and selection based on the application requirements.

Application Requirements

Finalists may be contacted for an interview. Students will be notified of any scholarship award at the South Stars Annual banquet.

Completed and signed application—emailed with required attachments by January 31, 2025. If mailed, please forward them to Chet Whitaker Memorial Scholarship Foundation at 300 Robert Street Burlington, WI 53105. Please make sure these items are postmarked by January 15, 2025, to make sure they are received by the January 31, 2025, deadline for documents.

A brief description of planned area of study and career goals
An Essay must accompany your application that is no longer than 500 words. Essay Question: Please describe how you feel the time you have spent playing hockey for the SOUTH STARS has prepared you for the next step in your life and has helped you to display the leadership, sportsmanship, and dedication skills that Chet displayed his entire life.
